You can just type it into something like notepad to get it uploaded. So yeah some software is required to get the design to where you need it to go, but technically you could design the whole thing on pieces of paper if you knew what you were doing. The websites that help you design websites basically just present this code in more user friendly fashion. If you go to any webpage and hit View>Source you can look at the code that's behind that page.
