Haven't smoked a cigarette in over 3yrs FWIW.

Also, Spain has its own national health that I'm eligible for as I'm paying resident tax over here.

And I get private cover through work both in UK and Spain anyhow.

But I see your point re smokers costing the health service. Total cost of smokers is less than the obese AFAIK (though I haven't got 2012 figures to hand), and smokers are already taxed through the rate of tobacco sales as well.
