Nah, not always. Think it refers to various things on record shops, maybe a smaller first print to judge demand, maybe a few released up front to a certain store or yeah, maybe white label/no artwork.

If the picture shows full artwork, it's a good bet that's what you'll get.

dunno why, it's usually the ordinary release
