From what i've heard (not being crunk enough to have experienced it first hand) it's grape flavoured cough syrup which contains codeine, occasionally mixed with sprite to make a drink, or in this case, drank.

Also called sizzurp, i think the entire chopped and screwed thing in hip hop came from this too (being off your tits on codeine that is)

As far as i know, most of our cough mixture in the UK doesn't contain codeine, which sucks. Still, a couple of co-codamol and it's all good.
